You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. SC2 self-certification forms for shorter periods of illness (up to seven calendar days) can be completed electronically.
If after this 7 day period you still need a sicknote a request can be made on Consult Online from Home - Causeway Medical Centre & Great Sankey Health Centre (webgp.com) and choose sicknote or you can request on via email at warccg.causewaymedical.centre@nhs.net.
