We accept new patients who live within our practice areas.   

There are two forms which must be completed to apply for registration: 

       1. Complete the pre-registration form :

YOU MUST PROVIDE YOUR NHS NUMBER, YOUR  PREVIOUS SURGERY CAN GIVE YOU THIS  INFORMATION.   (registrations without NHS numbers will not be accepted,  unless you are from abroad & registering with a GP for the first  time).

YOU MUST INCLUDE YOUR PREVIOUS ADDRESS IN THE UK &  YOUR PREVIOUS GP DETAILS.

IF YOU ARE FROM ABROAD & WE ARE YOUR FIRST GP, YOU MUST   INCLUDE  THE DATE YOU FIRST CAME TO THE UK  

            2. Complete the online medical questionnaire  

REGISTRATION REQUESTS WILL NOT BE ACCEPTED WITHOUT ALL OF THE DETAILS REQUESTED.
*YOUR APPLICATION WILL TAKE A MINIMUM OF 5 DAYS TO PROCESS, IF WE HAVE YOUR MOBILE NUMBER WE WILL SEND YOU A MESSAGE WHEN YOUR REGISTRATION IS COMPLETE, OTHERWISE PLEASE CONTACT RECEPTION AFTER 5 DAYS TO CONFIRM.
Our Policy on Medication

It is the policy of this Practice not to prescribe the following medication if it is not indicated:

Diazapam, Tamazapam, Nitrazapam, Lorazapam, Zopiclone, Zolpadine

New patients who are currently prescribed these items will be proactively and promptly weaned off with the support of our clinical team.
